About Rhinoplasties

Rhinoplasty can be performed to enhance the aesthetic appearance or function of the nose. This procedure involves restructuring the nasal bones, cartilage, and soft tissues to provide permanent improvements, with techniques designed to address both delicate and more prominent concerns.

The first step to enhancing your nose is attending a personal consultation at either our Manhattan or Cedar Grove office. During this consultation, Dr. Berlet will meet with you and ask several questions regarding your medical history, current prescriptions, concerns, and treatment goals. He will also evaluate your nasal structure and skin quality.

Dr. Berlet will use this information to determine your eligibility for a rhinoplasty and create your treatment plan. He will recommend the most suitable surgical techniques for your case and may even suggest additional procedures to further enhance your results. Throughout the consultation, Dr. Berlet will also take the time to answer any questions.

Preparation

When preparing for your rhinoplasty, you will be given a set of detailed pre-operative instructions to guide you. These instructions may vary depending on the details of your procedure, but each instruction should be followed to ensure a successful procedure and smooth recovery.

You may be asked to modify your medication regimen to avoid the use of blood-thinning medications and supplements. Alcohol and nicotine consumption should also be discontinued for several weeks before and after your procedure. You will need to pre-arrange transportation home from your procedure, as well as arrange for time away from your job and other commitments. It is also advisable to arrange for at-home care from a trusted friend or family member during initial recovery.

Procedure

At the beginning of your rhinoplasty procedure, your skin will be cleansed and sanitized before general anesthesia is administered. Dr. Berlet will then place incisions using one of two techniques: closed rhinoplasty or open rhinoplasty.

  • Closed Rhinoplasty: A closed rhinoplasty places incisions within the nostrils. This technique leaves essentially no external scarring in most cases.
  • Open Rhinoplasty: An open rhinoplasty places an incision on the columella, or the strip of flesh between the nostrils. This technique is typically used for more extensive corrections, such as nasal tip refinement.

Once the incisions are placed, Dr. Berlet will make the required corrections. He may excise tissue, reposition bone, or reshape your cartilage to create a narrower nasal base, refine the tip of your nose, straighten your nasal bridge, and more. Dr. Berlet can also perform any functional corrections if needed, such as correcting a deviated septum or addressing blocked airways.

In some cases, a spreader graft may be needed. This technique consists of placing thin strips of cartilage between the septum and the upper lateral cartilages. (3) This may be performed for both functional and aesthetic concerns, with grafts typically being taken from the ear, septum, or ribs. After Dr. Berlet closes the incisions, you will rest in a recovery suite until you are ready to go home.

Recovery

During your recovery, it is normal to experience swelling, bruising, or tenderness around your nose. These effects are temporary and will gradually subside on their own, but may also be managed with prescribed medications. You may return to work and other light activities after resting for approximately one to two weeks, but you will need to refrain from strenuous or physical activities until cleared to return.

You will also be required to wear an internal and external nasal splint throughout the first week of your recovery. This splint is designed to protect and maintain the new shape of your nose, and will be removed after approximately one week at a follow-up appointment. We ask that you attend each follow-up appointment so that Dr. Berlet may monitor your recovery progress and ensure your results are developing as planned. Any additional post-operative instructions will be provided during your consultation.

You may notice improvements in the appearance of your nose immediately after your procedure, but the full results of your rhinoplasty will become more apparent as any residual swelling subsides.
